Relationship between Pre-operative Bleeding and Perforation and Bleeding after Operations for Duodenal Ulcer.

چکیده
Bleeding from a proved or probable stomal ulcer at some time after an operation for peptic ulceration often seems to be preceded by haemorrhage from the original, usually duodenal, ulcer. This association has been noted in the past (Hurst and Stewart, 1929; Donaldson, Handy, and Papper, 1958), but satisfactory evidence in its favour is lacking. The case histories of patients admitted to the Central Middlesex Hospital with bleeding following an operation for duodenal ulceration have therefore been reviewed and the frequencies of both preoperative haemorrhage and perforation in this test group have been compared with those in a control group of an unselected consecutive series of operations for duodenal ulcer.

Previous studies have suggested that patients operated upon for duodenal ulcer who have a previous history of perforation or haemorrhage are particularly liable to haemorrhage after operation from a proved or probable stomal ulcer (Langman, 1965). The cause of this association is unknown but there is evidence that patients of blood group 0 are particularly prone to duodenal ulcer haemorrhage (L...
متن کاملGiant duodenal ulcers after neurosurgery for brainstem tumors that required reoperation for gastric disconnection: a report of two cases
BACKGROUND Despite the efficacy of pharmacotherapy for gastrointestinal ulcers, severe cases of bleeding or perforation due to gastrointestinal ulcers still occur. Giant duodenal ulcer perforation is an uncommon but difficult-to-manage pathology with a high mortality rate. We report two cases of giant duodenal ulcer perforation after neurosurgery for brainstem tumors that needed reoperation for...
متن کاملVagotomy and Pyloroplasty for Gastric Ulcer.
In a series of 68 patients with gastric ulcer who were treated by vagotomy and pyloroplasty there were no operative deaths, though one patient who received massive transfusions died 14 weeks later from hepatic necrosis probably associated with serum hepatitis. Four patients developed recurrent peptic ulceration requiring further surgical treatment. The clinical condition of all but three of the...
متن کاملI-23: Hysteroscopic Complications
Hysteroscopic complications are infrequent. Major complications include: 1. Uterine perforation that is the most common complication of operative hysteroscopy (0.8 - 1.6 %). The perforation rate is less during diagnostic hysteroscopy. 2. Bowel or bladder injury which is rare, but may occur in association with uterine perforation or as a result of use of electrical current. 3. Cervical laceratio...
متن کاملNon-steroidal anti-inflammatory drug ingestion: retrospective study of 272 bleeding or perforated peptic ulcers.
The ingestion of non-steroidal anti-inflammatory drugs (NSAID) in 272 patients with bleeding or perforated peptic ulcer was compared with 272 age/sex matched controls. A significantly higher proportion of patients with gastric ulcers had received NSAID than those with duodenal ulcers. Twelve of 90 (13%) patients admitted with bleeding duodenal ulcers had received NSAID compared with 11 of 26 (4...
